The CDC did not revisit this one through intellectual courage but because outside pressure finally made silence impossible.

Who benefits from unchanging policy? Drug manufacturers benefit from stable demand. Health bureaucrats benefit from simplicity, which looks clean in reports and avoids difficult conversations. State health departments benefit from one-size-fits-all rules that need no local judgment. The people who pay most attention are the ones with the least incentive to ask hard questions.

The committee's debate itself mattered less than the fact that debate happened at all. The two worldviews on display were not new: one treats universal recommendations as the safest path, the other asks whether universal means right. What changed was that the second view got a seat at the table. That should have happened years ago.

Institutions resist re-examination of old decisions. Admitting that a 30-year rule might need change means admitting that no one questioned it sooner. It means asking uncomfortable questions about how many other policies sit unchallenged for decades. The CDC still recommends caution on such matters. But once a committee agrees to debate, the real work begins.
